Arranging an Appointment

Office visits are by appointment only. If you are unable to keep your appointment, the courtesy of 24 hours advance notice is appreciated.

Ample time is allotted for each patient. Since emergencies are never refused, they occasionally disrupt the office schedule. This priority for emergency care ensures the highest quality care for all of our patients.

Insurance

Our office staff is happy to assist you in filling out all insurance forms. We will need pertinent information including the policy holder’s name, contract number and group number (if applicable).

Medicare assignment and private insurance will be accepted in most cases. The following insurances are accepted by us and are subject to change.

Prescriptions / Refills / Lab Results

For prescription refills and the results of laboratory testing, please call during regular office hours when we have access to your records. Dr. Gage has allocated a specific time of the day to personally return your calls and answer your questions.

Fees

To maintain quality care at a reasonable cost and to minimize the cost of billing and collection, we ask that you pay for services as they are rendered.

Under certain circumstances, special considerations may be extended. Please discuss any financial questions with our office manager.
